How local hospitals have responded to COVID-19

Just like COVID-19 has done with so many other industries, hospital and medical care systems also have needed to bob and weave, since the entire world changed around March.

AdventHealth Zephyrhills and AdventHealth Dade City president/CEO Amanda Maggard outlined the medical community’s COVID-10 response — and what the future may hold for the pair of East Pasco hospitals that are part of one of the nation’s largest nonprofit health care systems.

“It has definitely been an interesting year, hasn’t it? 2020 is a year I don’t think we’ll soon forget,” Maggard said, as the opening speaker at the Zephyrhills Economic Summit, held on Oct. 14.

The hospital administrator detailed many of the initial challenges when COVID-19 cases spiked in March, during the event organized by the Greater Zephyrhills Chamber of Commerce, and convened at Zephyrhills City Hall.

A major issue involved procuring as much personal protective equipment (PPE) as possible.

“We have had a lot of PPE to serve, but COVID really escalated the amount and types of PPE that we needed,” the hospital executive said.

As a result, the hospital supply chain system has been forever altered. Now, it emphasizes partnerships on domestically produced PPE, while moving away from a just-in-time delivery model for such equipment and inventory, she explained.

In short, the pandemic offered “lessons learned” on how health care conglomerates handle and manage workforce, products, equipment and so on.

“We will never go back to operating our supply chain the way we did before COVID,” said Maggard, who’s had her role since 2017. “We have much more of an inventory across our system today than we did before, and that will stay.”

Since the pandemic touched down in the spring, hospitals have stopped many elective surgeries and procedures, including outpatient physical therapy.

Instead of laying  off those employees, workers were re-deployed in any and every way possible. Some tasks included handling temperature checks, child care services, cafeteria and nutritional services, and even organizing fun activities for other team members, Maggard said.

“I’m so proud of the work that our team has done during this time,” she added.

Local hospitals are “continuing to learn and apply the latest clinical, evidence-based best practices, so we’re taking lessons from all of AdventHealth and around the country, and trying to apply those to make sure that we’re giving the best possible care to patients with COVID,” the hospital CEO said.

For example, the hospital is using Remdesivir, convalescent plasma therapy and “a number of other treatments, as we continue to learn and evolve,” Maggard said.

Remdesivir is a broad-spectrum antiviral medication; convalescent plasma therapy involves using blood from people who’ve recovered from an illness to help others recover.

Best practices to attack and treat COVID-19 have likewise involved ongoing learning because it began and still remains a relative unknown virus to the medical industry, Maggard said.

“We’ve been learning new things every day, and every week and every month,” she said, adding  “guidance has changed, as the science has changed and evolved.”

The entire AdventHealth system actively monitors the volumes of COVID-19 cases throughout its various hospitals. The “biggest surge” of cases was seen in mid-July, Maggard said, adding: “It’s declined since then, (but) we saw a little bump up recently.”

Going forward, the hospital system is continuing its myriad health and safety protocols — such as markers on the floor to remind people to maintain six feet of social distance;  temperature checks and screening everyone who enters the facilities every day; requiring masks; and regular deep cleanings throughout entire facilities, as well as other measures.

All patients who are COVID-19 positive, or suspected of being positive, are kept in a separate space — away from other patients — “to try to keep everyone safe,” she said.

Long-term health care consequences
A noteworthy consequence of the pandemic, Maggard said, is how many people have delayed routine or more serious health care issues — largely out of fear of going out to a public hospital or medical facility.

Between March and April, emergency rooms saw a 30% to 40% reduction in visits pertaining to heart attacks and strokes, Maggard said.

“The truth is, people were still having the same amount of heart attacks and strokes, but they were waiting and not coming to the ER,” Maggard said.

Because of those fear-driven consumer behavior changes, people who recently have been coming back to the hospital have more serious clinical issues, because they chose to delay care.

The same goes for routine screenings, primary care visits and procedures, such as colonoscopies, she added.

“My concern long-term with COVID is not just the immediate impact of COVID, but what is going to be the impact of health of folks around our country and around the world over the next few years, when we may have started to put off that routine care because we’re afraid to go be seen,” said Maggard.

“Don’t put your health care on hold. We want to continue to be here and not see your long-term health suffer because of not getting some of the screenings or preventative care that you may need today.

“If you’re experiencing any of those symptoms, you want to get into the hospital. We want to assure people that we are a safe place to be, we have a number of precautions in place to keep you safe, and we don’t want you to delay that care.”

Another related question is how newly jobless people will get their needed preventative services if they’ve been laid off and therefore lose their employer-sponsored health insurance.

“With the impact to the economy and the number of jobs being lost, what impact will that have in the coming 12 to 24 to 26 months?,” Maggard said.

Another looming question is how the hospital system’s financial losses amid the pandemic — due to postponing elective surgeries and decreased emergency room visits — will impact available capital in coming years. As a nonprofit, Maggard explained such capital would otherwise would be reinvested into the facilities for operating room expansions and technology and equipment upgrades to adequately serve in a growing community, such as East Pasco.

“There have been a number of financial losses,” Maggard said, “and that is something that we’re struggling with right now, is (how) our financial impact due to COVID is going to restrict the amount of capital we have available in these coming years,” she said.

Perhaps one of the positive developments in result of the pandemic is the increased accessibility and availability of telehealth services for physicians, primary care and even specialists.

Telehealth is the distribution of health-related services and information via electronic information and telecommunication technologies, allowing for long-distance patient and clinician contact, care, advice, reminders, education, intervention, monitoring, and remote admissions.

Simply, it allows patients to virtually see a physician or specialists without having to physically visit a hospital or medical office.

Telehealth, Maggard said, is “really, truly a positive thing in health care that could increase access for a number of folks.”

“That’s actually one of the really great things that has come out of COVID,” Maggard said. “The number of guidelines and allowances that came out from CMS (Centers for Medicare & Medicaid Services) are allowing more telehealth than we were able to do before, and we get reimbursed for that. I hope that’s something that stays out of COVID.”

Community development grants on the way?

Talks are underway in Zephyrhills to offer a greater variety of business grants within the city’s CRA (Community Redevelopment Agency) district.

At last month’s CRA board meeting, Zephyrhills CRA director Gail Hamilton pushed for a wider range of incentives to help encourage new business investment in the 500-plus acre district.

The district encompasses the center spine of the city, generally between Hercules Park to C Avenue, and from Zephyr Park to 17th Street. Within those boundaries are the following historic neighborhood districts: Hercules, Historic Jeffries, Historic Abbott, Moore’s Estate, Zephyr Lake, Oakside, and Plaza.

Hamilton referenced a 2019 market report from consultant GAI Community Solutions, which indicates that vacancy rates are down in the CRA district, but property values are dropping because older retail/office buildings are not being improved or rehabilitated.

Interestingly, however, Hamilton said rental rates in the district have increased because “there’s a short supply of those buildings that are habitable, that are in the right location, where somebody wants to be.”

The market report shows the majority of rentable office properties within the district were built prior to 1980, and the majority of retail properties built prior to 1990. Further, since 2011, no new office properties and just one new retail property have been built within the district.

The report showed the limited inventory will weigh on district activity in the face of a growing local economy, Hamilton told CRA board members.

“We need to address that we haven’t built any new buildings and what we have is old,” the CRA director said.

“We still have building stock that nobody wants to rent because of the issues with the building or where it’s located or how it’s chopped up, and so we might want to take a very focused look at some of those buildings and try to work with the property owners,” she suggested.

The city currently offers grants related to building exteriors, including a matching façade rehabilitation grant program of up to $2,500.

But, Hamilton thinks the program should be expanded to allows some grants related to interior projects, such as plumbing or electric improvements.

“I can do signage, I can help them with a window or door, but the grants don’t allow for anything in the interior of the building, so that’s something we may want to talk about,” she said.

Hamilton also suggested creating some type of kitchen remodeling grant, particularly for high-quality kitchen fire suppression systems that could be used for subsequent tenants, if needed.

The Zephyrhills CRA (Community Redevelopment Agency) area is a dependent special district in which any future increases in property values are set aside in a trust fund to support economic development and redevelopment projects, within the designated district.

She put it like this: “If that restaurant goes out (of business), you still have a restaurant and that investment the CRA makes putting the money in the building, because the building will always be there.”

The CRA director added she’s seen high demand from retailers and restaurateurs looking to lease newer or updated building spaces, even as small as 1,000 square feet, in the downtown area.

“Not a day goes by that somebody doesn’t call the CRA office asking about space — and, I don’t have it,” she said.

The Zephyrhills CRA includes about 300 business, with the largest concentration of employment within retail trade, health care services, and accommodation and food services.

The district’s largest employer is AdventHealth Zephyrhills (1,100 employees), followed by Florida Medical Clinic (850), Nestle/Zephyrhills Bottled Water (325), City of Zephyrhills (180) and Morrow Steel (90).

Another issue, Hamilton said, is the limited supply of affordable housing for middle-income workers, close to their jobs.

Hamilton said she’s had ongoing discussions with AdventHealth Zephyrhills CEO Amanda Maggard about the issue, talking about creating some type of joint residential grants “so that that workforce group from the hospital has an opportunity to live and work here in the CRA district.”

The city offers a $5,000 residential ownership incentive grant to homebuyers who purchase a home in the CRA district, and maintain residency for five years. The CRA awarded five such grants last fiscal year, Hamilton said.

Florida Hospital Dade City set for upgrades

A slew of upgrades are coming to Florida Hospital Dade City.

The hospital’s parent company, Adventist Health System, is investing $20 million into various infrastructure and technological improvements to the 120-bed facility located at 13100 Fort King Road.

The upgrades will include a new lobby and cafeteria, as well as renovated units and spaces, which will see new flooring and automated medical equipment.

Maggard said several of the upgrades will go into effect over the next 18 months, as other capital improvement projects are also being determined.

“We have some really exciting things that are about to happen,” she said.

In addition to facility renovations, Maggard also mentioned the hospital’s first-ever electronic medical records system, Cerner, launches on Dec. 2

The $5 million system is expected to make it easier for network providers to access and share information with the hospital.

“It helps us not just provide a more connected patient experience, but it helps us understand the data,”  Maggard explained.

Those aren’t the only changes coming soon to Florida Hospital Dade City.

Beginning in January, all of Adventist Health System’s wholly owned hospitals and hundreds of care sites across its footprint will adopt the AdventHealth name and logo.

In east Pasco, Florida Hospital Zephyrhills, Zephyrhills Health and Rehab Center, and Zephyr Haven Health and Rehab Center will also take on the AdventHealth title.

That announcement was made by the company in August.

The organization said the naming structure was made to better allow consumers to more easily distinguish AdventHealth’s care locations and services; it does not reflect a change in ownership or business structure.

Maggard said of the rebranding: “This is not anything other than we’re trying to be that one unified team providing that same care. We have the same vision, the same mission across all of our facilities.

“You wouldn’t’ know as a consumer that we’re all connected. We’re one large organization with one goal, one purpose: To provide this whole-person care — not just your physical needs, but your mental and your spiritual needs as well.”

Adventist Health System is one of the nation’s largest faith-based health care systems, with more than 80,000 employees across 50 hospital campuses and hundreds of care sites throughout 10 states.

Florida Hospital CEO settling into new role

It’s been a busy time for Amanda Maggard, since being appointed Florida Hospital Zephyrhills CEO more than a month ago.

Her first week on the job presented a unique and challenging experience, navigating the 149-bed regional medical center in the wake of Hurricane Irma, to ensure patients could be cared for at any time.

But Maggard came prepared, considering she’s dealt with four other hurricanes throughout her career in hospital administration.

“Our team here did a great job. Overall it went very, very well,” said Maggard, when she sat down for an interview with The Laker/Lutz News. “The main thing is we were ready to care for all the patients and we got the team members here to do it.

“Fortunately or unfortunately, it’s my fifth hurricane that I’ve been through,” she said.

Since that whirlwind of a week, Maggard has turned her attention to hospital operations.

She’s focused on “listening and learning,” building relationships with hospital staff, patients and members of the east Pasco community.

“I’m kind of in that road show of just meeting a lot of people, both inside and outside the hospital,” Maggard said.

Aside from attending various events and functions citywide, Maggard has been shadowing key departments within the hospital, working shifts in the operating and emergency rooms, and observing surgeries and other medical procedures.

“I just enjoy being on the frontline and learning as much as I can,” the new hospital CEO said. “If you don’t have that connection point, you’re not always able to see and hear what our patients are experiencing or what our team is working through.”

Maggard was appointed Florida Hospital Zephyrhills’ new CEO on Sept. 4, replacing Randy Surber, who, after nearly four years, was promoted to CEO for Florida Hospital Heartland Medical Center in Sebring.

During her first three months on the job, Maggard said one of her goals is to take a deep look at the hospital’s key service lines and facilities to determine possible improvements, based on the needs of the Zephyrhills community.

“What are the service lines that the community needs? What are those services that we could provide here that there’s enough of a need and a demand and that we can perform with great clinical quality?”

Finding ways to enhance the patient experience is another objective of Maggard’s, whether it’s through more personalized care or finding ways to reduce medical costs.

Said Maggard, “I feel very strongly that we’re here to serve the community and serve the patients. Our (Adventist Health System) mission is to extend the healing ministry of Christ, and I think if we’re not creating a great experience for our patients, we’re really not holding true to that mission. And if we’re not constantly looking for how we can do that better, we’re not living out our mission.”

Maggard, who comes from a family of nurses and healthcare professionals, also hopes to develop a culture at the hospital centering on responsibility, accountability and positive relationships, among others.

“I think some of the important elements of creating that ‘right’ culture are ownership,” Maggard said. “You’ve got to have a team that understands that they own their results, and they own their experience. It’s not someone else’s responsibility; it’s all of our responsibility. Part of that’s accountability, too. And it’s got to be accountability, from the bottom up and the top down.”

She continued: “I’m also a firm believer in, ‘life’s too short not to love your job’, so you’ve got to have a little fun in there, too. Patients feel that. I’m a big fan of Southwest Airlines and the culture they’ve built over the years, and they have a way to inject just a little bit of fun and personality into what they do.”

Steady community involvement also will be critical for Maggard, as the hospital is the city’s largest employer with more than 900 employees and 360 active physicians.

“I think its really important to be connected with the community because the community has a sense for what the needs. Typically, the hospital is one of the largest employers in the community, so it’s important that we’re looking for ways to give back and that we’re aware of the issues and how we can be involved with the community to improve it,” Maggard said.

Though just 34 years old, Maggard’s experience in healthcare administration is vast.

She’s worked for Adventist Health System—which operates Florida Hospital Zephyrhills and 44 other medical facilities — since 2004.

Prior to being named CEO of Florida Hospital Zephyrhills, Maggard oversaw the strategic development and operations for Florida Hospital Winter Garden, an outpatient facility. In advance of its opening in 2016, she led the facility’s design and construction, operational planning, community and physician engagement and team onboarding. Maggard’s responsibilities later expanded to include senior leadership for Florida Hospital Apopka’s ancillary and support services.

Her other stops include Winter Park Memorial Hospital, as assistant administrator and chief operating officer, and Florida Hospital Altamonte, as director or operations.

She also served as manager for training and consulting for the health system, as well as a marketing and foundation coordinator Florida Hospital Apopka.

In a release, Adventist Health System president/CEO Terry Shaw called Maggard “an effective and mission-focused leader who has excelled in engaging physicians, team members and the community in providing exceptional, whole-person care.”

Some additional thoughts from Amanda Maggard, new CEO for Florida Hospital Zephyrhills

Q: Long-term, for you, what are some of some of the most important aspects of the hospital CEO position?

A: “I think, for me, it’s a few things. Absolutely you’ve got to have great relationships with your physicians, because they’re also closest to the patient and they understand what the patients’ needs are for care and experience. You’ve got to create the right culture for your team. I am a firm believer in life is too short not to love your job and if you’re team doesn’t enjoy what you’re doing then the patients aren’t going to feel that and have a great experience, so we’ve got to set the right culture here for the team, and that has a trickle-down impact for everyone.
… I think getting out there rounding with the team, spending time with them, and really, we are a faith-based organization, and one piece of advice that I received before is to look at the role, not just as a CEO, but you’re really the Chief Spiritual Officer for your hospital, so I think that’s another aspect as well, looking at the spiritual health of the team.”

Q: Which aspects healthcare administration/operations do you most enjoy?

A: “I do really enjoy building that culture with the team. It’s very rewarding to me when we have a culture where employees enjoy what they do, including our physicians and where patients feel that difference when we’re involved with the community. We have a great reputation in the community and I enjoy improving results, so I do really love that you that you get to create that right culture, that culture has to then translate into better outcomes for our patients and a better experience. I really love being able to see that — what are the metrics that we’re using to monitor that performance and then how are we improving them.”

Q: What is the most challenging aspect of healthcare administration?
A: “I do think one thing that is challenging for us is how do we reduce costs. The healthcare environment’s payment models don’t fully reflect that you always get rewarded for that, but we still have to do the right thing for the patient, no matter what. But, figuring out how to reduce costs is a challenge.”

Q: Have you been monitoring potential healthcare legislation and how it might affect the hospital?

A: “The bottom-line is what happens in Washington D.C. is very important and will have a significant impact on us as much as what happens in Tallahassee. We have to figure out how to create great clinical quality for our patients, a great experience at a reduced cost to them, and I think those elements aren’t going to change, no matter what happens with some of the legislation. It’s kind of that triple gain that you hear about: better quality, better experience and lower costs; so I try to stay connected with that and try to learn, but I also try to buffer my team a little bit, because it can be easy to be distracted. I think those three things have to be our key focus, no matter what happens with the healthcare bill.”

04/24/2021 – Keep Pasco Beautiful Earth Day

Keep Pasco Beautiful will celebrate Earth Day on April 24 from 9 a.m. to 2 p.m., at Crews Lake Park, 16739 Crews Lake Drive in Spring Hill. There will be educational and upcycle vendors; a ladybug release; presentations by Croc Encounters and the Owl’s Nest Sanctuary for Wildlife; native plants for sale from the Nature Coast Chapter of the Florida Native Plant Society; document shredding from 9 a.m. to noon; a rain water harvesting workshop at 10 a.m. ($45 fee); and compost workshop at noon. Preregistration is required for both workshops, at KeepPascoBeautiful.org/earth-day. There also will be a free household hazardous waste collection with Pasco County Solid Waste. The first 50 people to bring a thermometer containing mercury will receive a $5 Publix gift card (one per vehicle).
