Pasco Sheriff’s Office hone crisis intervention skills

Pasco County Sheriff’s Office deputies recently spent a week learning how to use the power of words to de-escalate situations that have the potential to turn dangerous.

In one role-play scenario, Pasco Sheriff’s Office deputies Breanna Chandler and Lindsay Steward intermediated between a mother and her teenage daughter with oppositional defiant disorder.

In another scene, deputies Ricardo Ortiz and Jose Vazquez-Trujillo sought to calm a schizophrenic man who had paranoia and homicidal ideations about a next-door neighbor.

Other mock scenarios had deputies and corrections officers encounter inmates or juveniles with suicidal thoughts, or, deal with someone with severe substance abuse. There was even a situation that exposed deputies to someone with low-functioning autism.

There weren’t any takedowns, hands-on combat nor use of nonlethal force.

Rather, this type of police training simply required conversations to resolve problems.

For each participating law enforcement officer, the objective was to have a better understanding of mental health in crisis and to learn how to better respond to de-escalate a situation during a call for service.

It was all part of the agency’s recent Crisis Intervention Team (CIT) training program at Pasco Safety Town in Shady Hills.

The weeklong, 40-hour CIT program is a collaborative effort between the sheriff’s office and many local behavioral and mental health community partners. It is held four times per year within the agency.

Throughout the week, deputies received instruction on various subjects, such as signs and symptoms of mental illness, co-occurring disorders, addiction, dementia, trauma informed care, and juvenile and veteran response.

De-escalation techniques centered on maintaining positive body language and tone of voice, along with active listening skills to appropriately reflect upon a subject’s feelings.

Deputies then put those techniques to the test with role-play scenes, which were written out by a licensed mental health counselor in collaboration with a law enforcement officer. Volunteer actors played the various roles of persons in crisis.

The mock exercises required deputies to determine whether or not to detain a subject for an involuntary mental health evaluation, otherwise known as the Baker Act; provide a referral to various behavioral health services and resources; or, simply offer someone a shoulder to lean on and to listen to a subject’s personal problems.

One of the instructors and role play actors was Tom Kelley, a former law enforcement and military officer, who operates a consulting firm specializing in crisis intervention and crime prevention.

In traditional law enforcement academies, trainees are taught to enter situations with authority in voice and action.

In the crisis intervention program, however, Kelley instructed deputies to find a softer, more inviting tone when dealing with people undergoing a mental crisis.

Kelley put it this way to a classroom full of deputies: “Let them know that you’re there and you care. At that moment, they need to feel like you’re the most caring person in the world, because they are at their worst.”

Pasco Sheriff’s Lt. Toni Roach has been the agency’s CIT coordinator since 2016.

She said the objective of crisis intervention is twofold.

First, it’s to reduce potential injury to citizens and responding deputies.

Second, it’s utilized to help divert people with “potentially criminal behavior or disruptive behavior” away from the jail system and into a mental health system where they can get proper diagnosis and treatment.

Roach explained: “The idea is to use your words and de-escalate rather than quickly resorting to force when the people you’re interacting with become nervous or scared, and are apprehensive to do what the deputy asks them to do.”

The training is vital for law enforcement personnel, the lieutenant said.

Roach put it this way: “We want to make sure that we can serve our public and provide them with good resources, and then hopefully we interact with them in a positive manner, so that, especially people with mental illness who have a fear of law enforcement, we can reduce that level of fear and have a good relationship with our community.”

Twenty-nine sheriff’s office personnel graduated from the recent spring class.

Upon graduation, each deputy received a pin for their uniform indicating they’ve had advanced training in crisis intervention.

Roach said about 50 percent of patrol deputies are CIT certified, also noting the public is able to request a CIT trained officer in a call for service. The agency is striving to have all of its deputies CIT certified in the future, she said.

In addition to the sheriff’s office, organizations that assisted in facilitating the CIT program were the National Alliance on Mental Illness, HCA Florida Hospitals, Rogers Behavioral Health, North Tampa Behavioral Health, BayCare Health System, Gulf Coast Jewish Family and Community Services, and Veteran’s Affairs clinics.

Town hall sheds light on opioid overdose prevention

Cesar Rodriguez spent 17 years of his life as a heroin addict.

He overdosed on five separate occasions, only to be saved by first responders who administered him naloxone, or Narcan, an emergency treatment that counteracts the life-threatening effects of opioid overdose.

“If I didn’t have Narcan,” he said, “I wouldn’t be able to sit here and share my experience. I would’ve never had a shot to recover.”

Parent Lisa Conca also has observed the Narcan’s life-saving effects when her son overdosed on heroin.

“I had never even heard of Narcan until that day he overdosed and was taken to the hospital,” Conca said. “Narcan saves lives and gives our kids another chance to help them on the road to recovery.”

These comments came up during a community discussion on opioid overdose prevention on March 8 at North Tampa Behavioral Health, which is located in Wesley Chapel. The event was organized by the Alliance for Substance Abuse Prevention.

The panel discussion was part of a community-wide effort to provide education about opioid overdose and what individuals can do to save the lives of those suffering with the chronic illness of addiction.

Much of the panel addressed how to access and administer Narcan.

Narcan is the first and only FDA-approved nasal form of naloxone for the emergency treatment of a known or suspected opioid overdose. The medication helps blocks the effects of opioids on the brain and restores breathing.

Opioids include heroin and prescription pain pills like morphine, codeine, oxycodone, methadone and Vicodin.

“Often the reason (over-dosers) end up passing is because the brain does not have oxygen. This medication helps alleviate that,” said Szilvia Boos Salmon, a pharmacist with Tampa Poison Control.

Along with the nasal spray, naloxone is available in an injectable form.

The Pasco County Sheriff’s Office is one of a number of growing law enforcement agencies to carry naloxone for possible overdose calls.

Pasco Sheriff Cpl. Sherry Johnson-Tandy said the agency has had over 60 deployments of Narcan. Each deployment was successful in preventing an overdose-related death, she said.

In addition to reversing overdoses, the medication is also for deputies and first responders who may become exposed or have incidental contact to illicit fentanyl or heroin at a particular crime scene.

Johnson-Tandy demonstrated how to safely and effectively administer both the nasal spray and injectable forms of naloxone. Free samples of Narcan were later distributed to the audience.

Johnson-Tandy said it’s onset time is anywhere between 1 minute to 3 minutes.

“It works almost just like an EpiPen,” she said.

“The best thing about this is, if you give it to someone and they don’t need it, it can’t hurt them,” she said, adding if someone overdosed on cocaine or stopped breathing for another reason, it won’t have adverse effects.

The Narcan medication might be needed more than ever.

Opioid overdoses increased by roughly 30 percent across the U.S., in just 14 months between 2016 and 2017, according to a new report by the U.S. Centers for Disease Control and Prevention (CDC).

The CDC called the data a “wake up call to the fast-moving opioid overdose epidemic.” It recorded 142,000 overdoses in hospital emergency departments across the nation, between July 2016 and September 2017.

Although not all overdoses in the study were fatal, they are part of the unsettling toll of opioids. Nationwide in 2016, illicit and prescription drug overdoses killed 64,000 people.

The problem is growing locally, too, experts say.

Pasco County had the highest rate of hospitalization in the Suncoast Region for opioid overdoses, according to 2016 data from the Agency for Healthcare Administration.

Additionally, Pasco has tied for fifth highest out of the state’s 67 counties for drug overdose mortality rates over the last few years, according to county health rankings.

There were 165 overdoses in Pasco in 2017— matching numbers from the 2010 and 2011 prescription pill crisis, said Capt. Mike Jenkins, who oversees the narcotics unit for the special investigations division at the Pasco Sheriff’s Office.

There’s also been a gradual rise in fentanyl and heroin use in the county over the past year, he said.

Other topics covered during the session included how to prevent opioid addiction and avenues for long-term treatment of people who are dependent on opioids.

Speakers agreed it starts with education and advocacy.

“We can’t arrest our way out of this issue,” Jenkins said. “It takes comprehensive, strategic partnerships to really move forward.”

“Start in the schools early, and educate kids as to how dangerous mom and dad’s pill bottles are,” said New Port Richey-based attorney James Magazine.

Communities must find ways to effectively integrate recovering addicts back into society after they receive treatment, Magazine said.

Rochelle Zwicharowski, a support specialist with the St. Petersburg-based Recovery Epicenter Foundation, said those who have been rehabilitated need to share their experiences.

“There’s 23 million people in recovery, and how many people do you know? We’re too quiet about it. If you’re in recovery, don’t be afraid to speak out about it because we do have to smash that stigma,” she said.

Awards honor visionary leadership, and local businesses

Porter’s family accepted the award at the Pasco EDC 31st Annual Awards and Trade Show on Aug. 31 at Saddlebrook Resort & Spa.

The annual event recognizes local businesses, and their owners, for their contributions to Pasco County’s economic development.

The new award singled out Porter for his “passion, strong will, persistence and dedication,” according to a news release from the Pasco EDC.

The Porter family sold portions of Wiregrass Ranch to develop The Shops at Wiregrass, and more recently Florida Hospital Wesley Chapel. Land donations also were made for the Pasco-Hernando State College’s Porter Campus at Wiregrass Ranch; and, for a future sports complex at Wiregrass.

Pasco County commissioners recently approved a series of agreements that will lead to construction of the complex.

IC, the mascot for Florida Hospital Center Ice made an appearance at the Pasco Economic Development Council’s 31st Annual Awards and Trade Show.

There will be a 98,000-square-foot indoor sports facility, outdoor ball fields, an amphitheater and event lawn, trails and a playground. A privately financed Marriott hotel also is planned.

In addition, local businesses received awards in six categories.

Twenty-four businesses were nominated, and scored based on company growth, leadership in current business issues and corporate citizenship.

“We love having the opportunity to honor Pasco companies, and all 24 nominees have had such an important impact on economic development,” said Bill Cronin, in a written statement. Cronin is the Pasco EDC’s president and chief executive officer. “It was difficult for the awards committee to narrow it down, but the seven winners truly deserve this honor.”

The winners for Business of the Year were:

  • Express Employment Professionals: Small business
  • FACTS Engineering LLC: Medium business
  • TRU Simulation and Training Inc.: Large business

Honoring Pasco County’s local businesses

The Pasco Economic Development 31st Annual Awards and Trade Show will honor local businesses on Aug. 31 beginning at 5 p.m., at Saddlebrook Resort & Spa, at 5700 Saddlebrook Way in Wesley Chapel.

The event will have a new format, with a shortened awards ceremony, a brief report on Pasco County’s economic development progress, and a trade show.

Instead of a sit-down dinner, there will be a variety of hors d’oeuvres.

Another change this year is the introduction of the Pasco EDC Leadership Award, which celebrates a person in economic development who has had a remarkable impact on Pasco County and the community. The inaugural award goes to the late Don Porter, who is being recognized for his vision in helping to shape the Wiregrass area into a community where people work, live and play.

The other awards being bestowed recognize top companies in Pasco that excel in technology, innovation job creation, growth, capital investment and community development.

This year 24 nominees were singled out for their contributions to the business community.

Nominees received scores based on company growth, leadership in current business issues, and corporate citizenship.

Awards are given in the following categories: small (1-20 employees); medium (21-100 employees); large (101 or more employees); and startup/new within past 24 months.

Stigma poses obstacle to mental health care

People who have been diagnosed with bipolar disorder have a 20 percent risk factor for suicide, Dr. Marlene Hart said, during a community seminar at Pasco-Hernando State College’s Porter Campus at Wiregrass Ranch.

Despite that staggering risk, however, many people refuse to seek treatment for mental health problems.

That needs to change, Hart told the audience.

A shift in public perception about the problem could go a long way to help, she said.

“We have to fight stigma. It interferes with people getting treatment. It causes people not to come in,” Hart said.  “If there is one message that I hope people get — is that psychiatric disorders, including bipolar … are medical problems.

“The brain is the most complicated organ in the body,” Hart added. “Why would we think, for whatever reason, it doesn’t have imbalances? It’s not logical.”

There are about 5.7 million adult Americans who have been diagnosed with bipolar disorder, which is about 2.6 percent of the population, according to the National Institute on Mental Health.

Hart, who is medical director at North Tampa Behavioral Health, has worked with countless clients struggling to overcome the difficulties of managing bipolar disorder. She spent 18 years of her career in Dade City, working with patients who grew up and graduated from high school and college. She worked with them through deaths of close family members, through marriages and divorces.

She got into the field because she wanted to help. Initially, she trained to be a social worker, but later went back to get her medical degree. She has seen the challenges of mental illness up close because her sister was a schizophrenic.

When people are diagnosed with bipolar disorder, it is very common to find someone else in the same family who has been diagnosed with the condition, Hart said. “It is one of the genetically connected medical problems that we know of.”

Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in mood, energy, activity levels and the ability to carry out day-to-day tasks, according to the National Institute for Mental Health.

There are four basic types of bipolar disorder; all of them involve clear changes in mood, energy and activity levels. These moods range from periods of extremely “up,” elated and energized behavior (known as manic episodes) to very sad, “down” or hopeless periods (known as depressive episodes). Less severe manic periods are known as hypomanic episodes, the NIMH reports.

Hart said the condition can be difficult to diagnose, and often can go undetected for 10 to 20 years.

“How do bipolar patients come to health care? They may be depressed,” Hart said.

“So, it looks like a major depression,” she said.

They don’t often come in when they’re manic, because they’re having a good time — unless it is causes them problems, Hart said.

When people come in to be treated for their mania, it’s generally because they got themselves into trouble, she said.

“People get into a fight at Walmart because somebody didn’t move fast enough, or didn’t get out of their way,” she said. Or, “people get into impulsive behaviors, like shoplifting. It’s the thrill. The excitement.

“When it gets severe, people run naked in the parking lot,” she added.

Or, they may go on lavish spending sprees or make foolish business decisions, she said.

Or, they become involved in sexual situations that are not the norm for them, which can lead to relationship problems.

“You can imagine if somebody is impulsively engaging in sexual behaviors they do not normally do, that this could cause a problem for their long-term relationship,” Hart said. “We have people coming in all of the time that say, ‘You know I got involved in this thing, I don’t know why I did it.’”

Excessive drinking may be another sign.

“About 40 percent of the people are estimated, who have bipolar, use alcohol to try to get themselves to sleep or calm down.”

She points to Robin Williams’ portrayal of Mork, on the television show, “Mork & Mindy” as a classic example of manic behavior.

The movie, “Silver Linings Playbook” also offers valuable insights, Hart said.

“I thought it was a really excellent portrayal of bipolar disorder,” she said.

Symptoms of bipolar disorder

Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in mood, energy, activity levels and the ability to carry out day-to-day tasks.

People having a manic episode may:

  • Feel very “up,” “high” or elated
  • Have a lot of energy
  • Have increased activity levels
  • Feel “jumpy” or “wired”
  • Have trouble sleeping
  • Become more active than usual
  • Talk really fast about a lot of different things
  • Be agitated, irritable or “touchy”
  • Feel like their thoughts are going very fast
  • Think they can do a lot of things at once
  • Do risky things, like spend a lot of money or have reckless sex

People having a depressive episode may:

  • Feel very sad, down, empty or hopeless
  • Have very little energy
  • Have decreased activity levels
  • Have trouble sleeping, they may sleep too little or too much
  • Feel like they can’t enjoy anything
  • Feel worried and empty
  • Have trouble concentrating
  • Forget things a lot
  • Eat too much or too little
  • Feel tired or “slowed down”
  • Think about death or suicide

Pasco County’s development ready to take off

Dreams and visions for the redevelopment of Pasco County began more than two decades ago. Today, homes, shops, hotels, offices and medical campuses are coming out of the ground and redrawing the county’s future.

On March 1, developers of four of Pasco’s master-planned communities updated more than 100 people who came to the annual Meet the Developers breakfast. The Pasco Economic Development Council sponsored the event at the Marriott Residence Inn, off State Road 54.

In closing remarks, Bill Cronin, president of Pasco EDC said, “this is the right time, the right place for Pasco County…If nothing else, I hope you have that ‘oh, wow’ effect. That’s not the Pasco I knew.”

Speakers at the breakfast were Matt Call, project director at Starkey Ranch, which is being developed by Wheelock Communities; Tom Panaseny, vice president and general manager for Bexley by Newland Communities; J.D. Porter, president of sales and development of Wiregrass Ranch Inc.; and, Kartik Goyani, vice president of operations for Metro Development Group Inc.

“We’re all selling this dream and vision of where Pasco can go,” Call said.

Three years ago, Starkey Ranch, about four miles west of the Suncoast Parkway, off State Road 54, was largely still on the drawing board.

Today, Starkey Ranch anticipates having more than 300 new homes under contract in a year, Call said. So far, there have been more than 150 closings, he added.

The average price of homes being sold there is in the low to mid-$400,000s. The most expensive home — at about 4,800 square feet – sold within the past two weeks for about $874,000.

“Most are not first-time homebuyers,” Call said. “Most are moving-up buyers.”

Upcoming projects included widening Gunn Highway and extending the road into Starkey Ranch.

About one-third of a 20-mile trail system is built and will eventually connect with Jay B. Starkey Wilderness Preserve and the Suncoast Trail.

A business park, fronting State Road 54, is in the works. A town square is planned for 2018.

While Newland Communities is new to Pasco, its track record in Tampa Bay includes master-planned communities in Hillsborough County, including FishHawk Ranch in Riverview and Westchase in Town ‘n Country.

Panaseny described Newland as the largest private developer of planned communities in the country, with projects in 19 states, stretching from coast to coast.

The homebuilder’s first Pasco community is Bexley Ranch, in an area encompassing about 1,700 acres. It is located also on State Road 54, east of Suncoast Parkway.

The Bexley family is developing another 5,000 adjacent acres still under their control.

The Newland project is zoned for more than 1,700 homes; about 95,000 square feet of retail; and, more than 560,000 square feet of office, though that could increase to as much as 1 million square feet.

About five miles of a planned 13-mile trail system is built.

An elementary school now under construction is scheduled to open in August.

About 400 lots are available, with 400 additional lots expected to be ready by the end of the year. Twelve model homes are open for viewing.

Bexley also is planned for offices, shops and a hotel, as well as apartments and single-family homes.

A groundbreaking is anticipated in March for more than 300 apartments.

Access to Bexley currently is through the Ballantrae community to the east. But, within 60 to 90 days, Panaseny said Bexley Village Drive should open, with access directly off State Road 54.

The average home price is about $350,000.

“It’s been a strong market,” Panaseny said.

Upcoming projects include a medical facility, a Race Trac gas station and convenience store, and a 7-acre shopping center.

“We’d love to have a grocery store anchor the center, or possibly restaurants,” Panaseny said.

A Marriott hotel is expected to break ground in July.

There also is about 32 acres available for offices. “We very much want to bring Class A office space here,” he said.

Wiregrass Ranch is rebranding itself, with a new logo and a re-energized focus as new developments in the master-planned community emerge.

Wiregrass is the fastest growing community in the Tampa Bay area, said Porter, of  Wiregrass Ranch Inc.

The property is zoned for 10,500 homes; 2.7 million square feet of retail; 2.6 million square feet of office; and 600 hotel beds. A middle school and a high school have been built. Additional retail and office space are planned.

Next to The Shops at Wiregrass, plans are underway for an expansion that will include high-end shops and boutiques, a cine bistro, a grocery store and apartments.

Raymond James Financial campus is expected to start construction later this year on a campus that could have about 1 million square feet of office space and an 1,800-space parking garage.

Medical facilities are filling up the surrounding area. They include Florida Hospital Wesley Chapel, Florida Medical Clinic and North Tampa Behavioral Health.

Beach House is an upscale assisted living facility under construction.

“You’re going to continue to see good, smart growth because it’s an environment people want to be in,” said Porter.

Metro Development Group is building a master-planned community at Epperson Ranch, which is part of a state-approved pilot program to create a Connected City corridor built with technology innovations. At build out, Metro is projecting about 37,000 homes; 12 million square-feet of office, retail and technology manufacturing; 100 miles of traffic lanes for self-driving vehicles; and two Crystal Lagoons.

The first lagoon at Epperson Ranch is a 7-acre, manmade pool of crystal blue water under construction at Epperson Ranch, off State Road 52 at Overpass Road. To the north, a second community, known as Mirada, is also planned with a larger Crystal Lagoon.

Metro controls about 35 percent of land within Connected City, which sets aside about 7,800 acres in northeastern Pasco. Metro and other developers are expected to build master-planned communities embedded with cutting-edge technology, and focused on creating technology driven jobs and new businesses.

A groundbreaking for the Crystal Lagoon at Epperson happened in February. The lagoon is expected to be complete by the end of 2017. In the first weeks, 15 homes have sold, and there is a waiting list of more than 1,000 people who want to buy, said Kartik Goyani, vice president of operations for Metro.

Growth continues to transform Wiregrass Ranch

Once known for its cattle and citrus groves, the Wiregrass Ranch area of Wesley Chapel is now being viewed as a place where people can live, work, shop, learn and receive medical care.

During the past decade:

  • Pasco-Hernando State College opened Porter Campus at Wiregrass Ranch, giving Wesley Chapel students their first opportunity to seek a four-year college degree in their own backyard.
  • Florida Hospital Wesley Chapel opened, and the demand was so great that the hospital expanded — well ahead of expectations.
  • Florida Medical Clinic opened, on land situated near the hospital.
  • North Tampa Behavioral Health opened, and has been so successful that it is already undergoing an expansion.
  • Financial services giant Raymond James closed on the purchase of 65 acres of land and is expected to break ground later this year on a satellite campus, across from the college.

And, that’s just part of the story, according to J.D. Porter, president of sales and development for Wiregrass Ranch, in Wesley Chapel.

Besides the health care, education and commercial projects, residential growth is going strong, too, Porter said.

Wiregrass Ranch — with 340 housing starts — was No. 1 in housing starts in the period ending Dec. 31, 2016, according to a report by MetroStudy.

FishHawk Ranch, in southern Hillsborough County, was second on the list with 324 starts.

Other Pasco County developments among the top five were Long Lake Ranch, at fourth place, with 227 starts; and, Starkey Ranch, at fifth place, with 219 starts.

Porter expects residential growth to continue to flourish in Wiregrass Ranch.

He pointed to a contract on a large tract of land for a new active adult community that’s expected to close this year.

The development, which will have around 1,000 units, will fill a niche in the market, Porter said. “Nowhere in Wesley Chapel is there really an age-restricted community,” he said.

It will be “highly amenitized,” he said, and will be designed for people who are 55 and older.

Another residential development will offer row houses, and will appeal to people who are looking for a maintenance-free lifestyle, he said.

More commercial growth is on the horizon, too, Porter said.

North Tampa Behavioral Health, on State Road 56, is in expansion mode.

A 15,000-square-foot medical office is going up next to Florida Medical Clinic, he said, noting the bottom floor will be occupied by an orthodontist.

He expects that to open within the next seven months to nine months.

“They’re turning dirt like crazy right now, which is fantastic,” Porter said.

The medical office market is hot, Porter added.

“We probably get five to 10 calls a day,” he said.

It’s possible that another 100,000 square feet to 150,000 square feet of medical office could be added, he said. “It’s going to be limited by size.”

Porter also noted that his family is actually doing a strip center of its own on State Road 54, which will include restaurants and businesses which cater to local needs.

And, while there’s been a considerable amount of interest about when Raymond James will break ground and bring new employment opportunities, Porter said other major players are also eyeing the Wiregrass Ranch area.

Wiregrass is also on the shortlist of a couple of other companies that have equal, if not greater, name recognition as Raymond James, Porter said.

“Now that we have these foundations, it just ramps up to a different level, which is exciting,” Porter said.

Wiregrass Ranch names COO

Scott Sheridan is a familiar face at Wiregrass Ranch.

As senior vice president of King Engineering Associates Inc., he has been involved from the start in the master-planned community’s development. Since 2004, he has taken on various roles with the planning, engineering and permitting of the project.

Sheridan now is putting on a new hat for the Wiregrass Ranch development company Locust Branch LLC.

He is the company’s first chief operating officer, according to a news release from the development company.

His goals as chief operating officer will be to oversee the upcoming growth at Wiregrass Ranch, as it moves into a new phase of development.

“Under (Scott’s) leadership and expertise, we are confident that Wiregrass Ranch will thrive as the region’s premier master-planned development,” Bill Porter, president of Wiregrass Ranch, said in the release.

The construction of the satellite campus of Raymond James Financial is among the future projects in the development of regional impact.

The financial services company closed on a land deal in September to buy 65 acres of ranch property from the Porter family.

The Fortune 500 financial giant could build as much as 1 million square feet of offices near State Road 56 at Mansfield Boulevard, and bring hundreds of jobs to Pasco County.

Wiregrass Ranch is a 5,100-acre mixed-use community in the Wesley Chapel area. It is home to The Shops at Wiregrass, Florida Hospital Wesley Chapel, Florida Medical Clinic, North Tampa Behavioral Health and the Porter Campus of the Pasco Hernando State College.

Sheridan is a Florida registered landscape architect and an active member of the Urban Land Institute, a nonprofit research and education organization. ULI provides a forum for more than 40,000 members in the real estate and land use professions.

Sheridan shares his expertise with the ULI’s statewide Urban Development and Mixed Use Council.

In 2014, Governor Rick Scott appointed him to the Tampa Bay Regional Planning Council for Hillsborough County.

Sheridan has a bachelor of science degree in urban and regional planning from Cook College at Rutgers University.

He worked for more than five years at Heidt Design as a landscape architect. He later worked in the land planning department at King Engineering for more than 14 years, and was a member of the board of directors.

Looking for tools to help patients help themselves

When it comes to treating patients with behavioral health issues, including Post Traumatic Stress Disorder, Dr. Laura Bajor is always on the lookout for new tools that could help.

She sees the potential of emerging technology.

But Bajor, who is medical director for the CORE program at North Tampa Behavioral Health, 29910 State Road 56 in Wesley Chapel, isn’t into technology for technology’s sake.

Organizations that engage in that approach, she said, “tend to create a ‘flavor of the month’ atmosphere.’”

Ultimately, that’s counterproductive, Bajor said, because “that actually ends up alienating folks from trying new things, because they’ve lost faith in the actual benefit of new technology.”

Instead, Bajor believes that “technology and research are most useful when they improve a patient’s investment in their own health, and their quality of life.”

She’s constantly on the lookout for new tools, or collaborations, to help her find the best ways to help the patients she’s treating.

“In my opinion, one of the absolute worst conversations to have with a patient is to have to sit across a desk from them and say ‘We’ve run out of treatment options for you, and we’re not working on anything new. Good luck.’

“The real aim of health care is not that they remain our patients, it’s that they function well on their own without us. So, we can move them toward that,” Bajor said.

Along those lines, “we’re using technology to assess sleep, activity level, change in heart rate throughout the course of the day, and a few other parameters,” she said.

This permits a more focused approach to prescribing medication and planning treatment, she said.

The idea is to be able to show patients evidence regarding their progress or lack of it, which helps save time and reduces frustration, she said.

Technology also is used to help patients learn how to control their own stress and anxiety levels, which helps them regain a sense of control and self-confidence, she said.

Bajor said she began using emerging technology about four years ago, with a series of small innovation grants in a clinic at the VA in Boston.

She was able to pilot the use of different kinds of fitness, sleep, and stress trackers with her staff and patients. That eventually spread into a partnership with the Basis division of Intel, who worked to equip patients from about 15 different clinics and programs with their gear.

She believes part of the reason she was recruited to become the medical director of the CORE program at North Tampa Behavioral was because of those experiences and skills.

Sleep plays a significant role in a patient’s health, Bajor said.

“The first paper I ever published was about tracking sleep first rather than hitting people with very high-caliber meds,” Bajor said.

When someone comes into the clinic and sleep is part of their problem, they are put on a tracking system for a couple of days before any medication is prescribed, she said.

“Is the problem that they’re not going to bed until 3 in the morning? Is the problem that they can’t fall asleep? Or, is it that they’re waking up 10 times?

“We would actually use different treatment approaches, depending on which or all of those problems they have,” she said.

“A person, once they’ve been sleeping, you can probably use much less medicine,” she said. That helps to avoid prescribing medication that can affect their functioning during the day, and reduces potential for side effects.

“All of these things have side effects,” she added.

While in Boston, Bajor said she worked with top-notch researchers and clinicians from the Harvard and Boston University systems, she said.

“I worked mainly in Ann Rasmusson’s lab at the National Center for PTSD, where there is a major focus on using exercise, cognitive therapy, and other novel approaches to calm down the neuroendocrine system, get folks’ frontal lobes back on line, and in doing all that, help PTSD patients get back in control of their anxiety,” she said.

“Ann and her crew have continued to be generous in providing advice about how to translate these ideas into our CORE program,” she said, where exercise, yoga, diet and other approaches are being used to treat veterans.

“There’s an emerging parameter called heart rate variability,” Bajor said. “It’s kind of the newest thing in physiologic tracking. It’s the rate at which a heart rate changes.

“We’re watching that with guys who are doing PTSD therapy,” she said.

“We can tell: Should we back off a little, or should we try harder?

“There’s actually NFL coaches and Olympic coaches that are using the same HRV (heart rate variability). They’ll say, ‘Well this guy should take a day off from weightlifting.’ Or, ‘We should push this guy harder, he’s not going hard enough.’’’

“We can kind of do the same thing,” she said.

Bajor also noted she’s received help from a number of other experts in the areas of research methods, dissemination of innovation and day-to-day management of new ideas in a clinical setting.

“The trajectory of my medical career has been a little unorthodox,” Bajor said.

She went from being a student at the Naval Academy, to becoming a search and rescue pilot, to being a Department of Defense engineer. She left that job to attend medical school.

In addition to her current work, she also belongs to “Tampa Hackerspace,” a group she describes as an eclectic group of brilliant folks who have pooled their resources into a space where members have access to things like 3-D printers, laser cutters, full metal and wood shops, and the expertise of leaders in their various fields.

She just joined the group in the summer and already has ideas that will be used in her clinic, she said.

“Making those connections and sharing knowledge. There’s no way to measure that. It’s just invaluable,” said Bajor.

She welcomes opportunities for collaborations that will lead to better care for patients.

Tackling problems of opioid abuse

A panel of local people working on the frontlines to reduce the problem of opioid drug abuse gained access to the nation’s drug czar last week in a Pasco County roundtable session initiated by Republican Congressman Gus Bilirakis, of Palm Harbor.

No certain solutions were offered during the May 3 discussion with for Michael Botticelli, director of the Office of National Drug Control Policy.

But panelists and people in the audience identified several areas needing improvement

They said:

  • The stigma surrounding drug addiction must be reduced
  • Parents need education to help prevent their children from becoming addicts
  • More tools and better access to care are needed to help people overcome addiction
  • Residential treatment programs should not be limited to 28 days
  • Innovative and creative approaches should be encouraged and shared
  • More community support is needed to help those who have completed treatment programs to avoid a relapse

Panelists said they appreciated the opportunity to talk directly with Botticelli.

Doug Leonardo, executive director of BayCare Behavioral Health put it like this: “I feel like I’m sitting next to a rock star. For folks in the field, this is the individual who has the president’s ear on policies related to substance abuse for this country. So, it’s really a big deal.”

Data from the Centers for Disease Control and Prevention show that opioids – a class of drugs that includes heroin and prescription pain medications – were involved in 28,648 deaths in 2014.

“We don’t have our arms around addiction and it continues to get worse,” Leonardo said. “It feels like two steps forward, one step back, sometimes.”

Part of the answer lies in changing the perception about people with addiction problems, speakers said.

“We’re talking about a brain disease,” Leonardo said. “We don’t put people in jail for having diabetes.”

Reducing that stigma can help lead to earlier intervention.

“We all tend to look the other way,” said Kelly Mothershead, a panelist who lost her only son to a prescription pill overdose. “You don’t want people to think you have a child addicted to drugs.”

Decision-makers often don’t understand the scope of the problem, said Nancy Hamilton, president and CEO of Operation PAR, an agency that screens about 30,000 people a year in seven counties.

“We admit about 14,000 into our continuum of care and we have over 4,000 people who are medicated-assisted treatment,” she said.

She hears decision-makers say, “They had their chance at treatment.”

They don’t realize that people addicted to opioids relapse most often, Hamilton said. “So, they may need two or more bites of the apple to put all of their act together.”

“We had a lot of clean-cut kids coming back (from the war). Guys who had joined the Marine Corps out of Catholic school and had been Eagle Scouts and football players, coming back getting paper bags full of opioids at Walter Reed (National Military Medical Center), showing back up in Dorchester and Southie (South Boston) and as soon as they ran out of the opioids, were targeted by heroin dealers that they had gone to high school with.

“It was a bit of an eye-opener to see such clean-cut guys overdosing and seeing their boot camp pictures in their obits and at their funerals.

“It really taught me that this is not a character issue or a class issue, it’s a human issue,” Bajor said.

The problem must addressed through enforcement and treatment, Botticelli said.

The Pasco County’s Sheriff’s Office agrees.

The law enforcement agency takes aim at the supply side, but also recognizes that it can’t arrest its way out of the problem.

It deals daily with problems caused by addiction.

The county jail is “probably the largest detox facility in the county,” said Capt. Chris Beaman, of the sheriff’s office.

It is working with BayCare to increase intervention.

When responding to a call, sheriff’s deputies sometimes encounter people who have hit rock bottom, Beaman said. “Maybe, at that time, we can get them to make the decision to get treatment.”

Parents need to learn what to look for and need advice on prevention strategies, and it should be offered at work places, so parents have a chance to receive it, Mothershead said.

“We have to educate before it happens,” Mothershead said.

More education is needed, agreed Pasco County Sheriff Chris Nocco.

“Right now, if there is any parent who doesn’t understand what SPICE is, and they think it’s something in their cabinet, we’re in trouble,” Nocco said. Abuse of SPICE, which are synethetic drugs, is growing.

Botticelli asked Leonardo to describe what he sees “as critical ingredients to get people to long-term recovery.”

Leonardo said ingredients leading to long-term recovery include some element of support, either family or someone like family; access to appropriate type of treatment for the individual; and, a supportive environment after treatment, he said.

Botticelli agreed: “We want a continuum of care, not just short-term treatment. This is a chronic disease and people need long-term care

“We have to make our communities safe and supportive for people in recovery,” said Botticelli, who has been in recovery himself for more than two decades.

Innovation and creativity are important, too, Leonardo said.

“Creativity is key because nobody is going to throw money at this problem,” Bajor said.

Closer collaboration would help, Bajor said. “We would love to network with other providers in the area to close some of the gaps that our clients tend to fall into.”

Botticelli asked Judge Shawn Crane: “What do you see as kind of the unique needs of veterans?”

Crane replied: “What we have really learned is that veterans respond better in groups of veterans.”

That’s because veterans have a unique set of experiences and a lingo that’s all their own, Crane said.

“When you are a Marine recon sniper, there’s not much of a job for you when you come back out of the service,” the judge said.

“I took people who were in Drug Court and put them into Veterans Court and into Veterans groups and they just flourished. They just felt so much at ease,” Crane said.

“We do need to look at these guys as being a little bit unique,” agreed Bajor, who was a Navy pilot. The North Tampa Behavioral Center has a program designed specifically for veterans.

“They push each other more than we push them,” Bajor said.
