Seminar sheds light on stigma

The old adage, “sticks and stones may break my bones but words will never hurt me,” may not be exactly true — at least for those who are working to overcome alcohol and substance addiction, mental illness and cultural competency issues.

In other words, hurtful words do matter.

That was the consensus of a group of panelists affiliated with the Pasco County Alliance for Substance Addiction Prevention (Pasco ASAP), a nonprofit organization working to mobilize the community to prevent substance use by implementing evidence-based strategies that produce environmental-level change.

The panelists spoke at a June 1 webinar, titled “Change The Language ASAP.”

The webinar sought to bring awareness to the role stigma plays in addiction, and how everyday words and phrases may have an influence regarding whether someone seeks needed help.

The hourlong discussion was moderated by Pasco Sheriff’s Office Cpt. Toni Roach, who oversees the agency’s Behavioral Health Intervention Team (BHIT) unit.

Stigma experienced everywhere
Cesar Rodriguez understands the problem better than many, as someone who has been in recovery for seven years, after battling a heroin addiction.

He has seen dark days, including an overdose that nearly killed him.

Now, he’s general manager at Sun Coast Roofing and Solar and is proudly married, with two children.

He’s also been a Pasco ASAP volunteer since 2015. He helps with fundraisers, town halls, and other outreach events.

Although he’s in a good place today, it didn’t come without constant struggles due to stigma, he said.

Before ultimately finding his path to treatment and recovery, Rodriguez told those listening in that derogatory terms, such as “junkie” and “alcoholic,” thrown at those with addictions can morph into a self-fulfilling prophecy — creating barriers that can discourage people from seeking help.

“You begin to believe the labels, and you think you’re not worthy, because you feel like you’re not worth the help,”  Rodriguez explained. “You start believing what everyone’s called you, you apply the label to yourself, ‘Well, I’m just a junkie, I’m not worth it, this is what a junkie does.’”

Stigma shows up in the real world, too, when individuals in ongoing recovery are looking for work.

With multiple felony convictions and a spotty work history, Rodriguez struggled for months, trying to find a job or even land an interview.

“I got door after door shut in my face,” Rodriguez said. “It was really disheartening.”

The increasing use of online applications also prevented face-to-face meetings with hiring managers, where life circumstances could be illuminated in more detail.

“We live in a time where almost every application is digital, so, if you look at me on a digital application with no opportunity for me to present myself and put no personality or anything to it, I just get shifted out of the pile,” he said.

Finally, he was hired for an entry-level labor position with Sun Coast Roofing.

Shame and bias toward former addicts is pervasive on social media, too.

Rodriguez has witnessed online users condemning and humiliating professionals and local businesses owners who are anonymously seeking help via 12-step recovery programs. People use social media to post old mugshots and share past criminal histories.

Said Rodriguez: “I have friends that are business owners that are also in recovery, and I see it used against them in social media, ‘Hey, did you know the guy that owns this business, here’s his mugshot,’ and people post it on social media.

“To have their mugshots paraded around Facebook, saying, ‘Don’t let this person in your house, he’s a drug addict with a record.’

“Well,” Rodriguez continued, “if you’re not a strong person, that might send you running, that might send you back out again.”

Recovery is hard, he said.

It’s particularly tough when someone has been successful in recovery — built a business or a brand, and rebuilt trust — and someone comes along to tear it down — “just out of petty jealousy or ignorance,” he said.

Stigma even exists within the recovery community, said panelist Toni Reynolds, a certified peer recovery specialist who’s also in long-term recovery.

In particular, there can be judgmental attitudes toward people on medication-assisted treatment used to treat opioid addiction, like Suboxone, Subutex, Sublocade or Vivitrol.

Panelists said these medications are used by many as critical first steps in the recovery process.

“There’s still a lot of tension when people announce they’re on that (medication), and a lot of times they’re not welcomed into traditional 12-step meetings. I think it really hinders their ability to feel like they’re part of a community,” Reynolds said.

She said she personally experienced pressures among coworkers at a former workplace, while she was in a recovery program.

“It was kind of frowned upon that I was in recovery,” Reynolds said.

“Some people that I worked with weren’t supportive, and they almost encouraged me to party with them, and it just wasn’t something that I wanted to do with my life,” she said, noting she was not willing to go backwards.

Stephanie McCann has been in recovery for four years now.

Overcoming addiction and a jail stint, the 32-year-old panelist assists others as a recovery support specialist for BayCare Health System.

McCann was an IV drug user for 12 years, abusing opiates and methamphetamine among other substances, before undergoing residential treatment.

She agreed with the assertion about negative perceptions toward medicated-assisted treatment — including medical marijuana — within recovery communities, such as Narcotics Anonymous and Alcoholics Anonymous.

While considerable stigma remains in those rooms, McCann said, she’s also recently observed increased understanding of the methods others are using to get clean.

“I think it’s a work in progress and it’s getting better,” McCann said. “I think people are getting a little bit more open to the fact that not everyone’s path to recovery is the same, and that’s OK, because ultimately, if they stride to recovery, what path they took is not necessarily as important.”

The speaker also noted that people often are reluctant to be open about issues surrounding substance abuse and mental health because of stigma.

As an example, McCann said her mother once displayed embarrassment in a doctor’s office when the daughter revealed to nurses and physicians she was attending Narcotics Anonymous groups.

“It really showed me that like older mindset with mental health and substance abuse, that it just wasn’t something you talked about in front of other people,” said McCann.

McCann, however, refuses to stay silent regarding her own assorted life experiences.

“I’ve totally embraced it,” she said.

She added: “Recovery is so much of who I am now, and the path that I’ve taken through addiction has made me the person that I am now, and I’m just not able to be quiet about it.”

Stigma surrounding substance abuse and mental illness also seeps into medical profession, said Dr. Philip Richmond, who serves as medical director at Country Oaks Animal Hospital in New Port Richey.

Richmond freely offered up his past challenges with alcohol since high school, and having suicide ideations back in 2008 upon entering the field as a “newly minted” veterinarian amid stress and other factors.

The panelist referenced a study that claims around 12% to 15% of physicians will encounter an alcohol or substance use disorder. He also referenced a Centers for Disease Control and Prevention (CDC) study that reveals veterinarians are about 3.5 times more likely to die by suicide than the general population.

Yet, these problems remain “very stigmatized” throughout the medical community, Richmond said, as health professionals feel they must resolve issues themselves, without outside help.

“We (as medical professionals) somehow think that we should be able to outthink things, that we should be able to tough it out,” said Richmond.

“That’s what almost led me to not being here, was thinking, ‘It shouldn’t happen to me, look at all these things I’ve done in my life, I should be able to overcome this by myself,’ and I 100% could not.”

In his case, colleagues intervened. They recognized what was happening and got him into an inpatient treatment facility, which saved his life.

He emphasized the need for compassion and understanding for those battling addiction and mental illness.

He cited a 2015 CDC study illustrating how people with four or more traumatic childhood experiences are seven times more likely to have an alcohol use disorder, seven-to-10 times more likely to have a substance use disorder, and 12 times more likely to take their own life.

“Things that happened to probably all of us on this panel, before we had any say in it, increased our chances exponentially of having a use disorder,” Richmond said.

That, he said, has been one of the most powerful things he has learned.

Pasco experiencing recent spike of overdoses

Since the COVID-19 pandemic began, the Pasco County Sheriff’s Office (PCSO) has reported a record-setting spike in drug overdoses.

A lethal dose of the painkilling opioid Fentanyl is 2 milligrams, almost imperceptible, but the PCSO Narcotics Unit removed 435,000 lethal doses of the drug from the county’s streets during a six-week period.

The story doesn’t stop there.

Aided by a federal grant, the PCSO has established a Behavioral Health Intervention Team, which helps citizens with mental-health issues or substance-abuse problems. Members of the 16-person team contact overdose survivors within 48 hours of the incident, then connect them with community rehabilitation partners such as BayCare.

According to Sheriff’s Office records, Pasco County overdoses reached an all-time monthly high of 167 (37 fatal) in May, then maintained a steady pace in June (146, 18 fatal); July (140, nine fatal); and August (147, 24 fatal).

From 2017-2019, the largest overdose total in any month was 89.

In 2020, there were startling overdose upticks in May (297% increase), June (239% increase), July (204% increase) and August (234% increase), compared to the average of those month’s totals in the previous three years.

But, to Pasco County Sheriff’s Capt. Toni Roach, the key number is 49. Those are the people who have been placed under the care of a substance-abuse program because of the intervention team’s work.

“I know that doesn’t seem like a large number, but we’re running at about a 25% commitment into a program (after intervention),’’ Roach said. “That’s (49) people I know who aren’t going to potentially overdose and die in our community. That’s because the detective has been able to engage them and help them get appointments with behavioral health providers.

“We’ve created a path, giving these overdose subjects a golden ticket, getting them to the head of the line for services. These overdose subjects, they overdose, they almost die and they want help. “The overall effects of withdrawal and addiction manifest themselves within the first 24 hours. That’s why it’s crucial to get to them before they use again. If we get them into services quickly and reduce the withdrawal effects, they’re going to be more successful in staying,” she said.

Sometimes, it’s difficult to follow through, though.

Roach said the intervention team has worked with approximately 1,000 overdose follow-up assignments in the last year. About one-third of the subjects couldn’t be located, potentially because they were homeless, transient or intentionally avoiding authorities.

For subjects who are located, about half of them are interested in services. Most of the time, Roach said, they don’t know where to start.

“We have that hard conversation and tell them, ‘This is your rock bottom, and we’re here to help you,’” Roach said. “We help take away all the excuses. We do all the legwork and take people to their first appointment. If the process is intimidating or they don’t have transportation, we get them that help.’’

Since the COVID-19 pandemic began, that help has been needed more than ever.

“We definitely saw an increase in the overdose numbers and there were multiple potential reasons,’’ Roach said. “Some of it was stimulus related. When the stimulus checks came out, some people partied like it was 1999. The substance abuse community relies heavily on peer support and interaction. So when we stopped meeting in person, churches closed and things went online, it wasn’t the same as having that human interaction.

“The whole aspect of social isolation has been a huge part of the pandemic and overall mental health. No doubt, isolation has played into it in a very big way,” she said.

Fentanyl also plays into it in a very big way. It’s the drug of concern for local officials.

“It’s extremely cheap and it’s extremely addictive because of its potency level,’’ Roach said. “It creates a bigger problem for us. The things that seemed more benign by comparison, such as marijuana, can now overdose and kill you because it’s now laced with something that is very fatal and highly toxic. It throws our entire community into a new playing field.’’

But, the intervention team continues to make headway.

“We’ve had a lot of great wins during this process,’’ Roach said. “As time goes on and we learn more and more, I think it will be even more effective.’’

Autism ID cards aim to help with policing

The Pasco Sheriff’s Office has another tool, aimed at helping its deputies when responding to situations involving residents with autism spectrum disorder (ASD).

The law enforcement agency has introduced an autism identification card program, otherwise being called Autism Assistance Cards.

The front of the cards has some general information to help deputies communicate with individuals with autism. For instance, deputies are advised to display calm body language and to allow for additional time for replies.

The back of the cards, meanwhile, offers space for an individual’s identifying information, such as name and address, and information on the person’s conditions, such as whether they are verbal vs. nonverbal.

The roughly 4-inch x 6-inch cards were designed with the help of River Ridge High School’s graphic arts class.

The ID cards are being distributed to individuals with autism, so if there’s an encounter, the card can be handed to deputies to help them know how best to work with the individual.

Cards have been provided to caregivers to share them with a loved one who has autism. They’re also being sent to special education teachers in Pasco County Schools, who can pass them along to students’ families.

Pasco Sheriff’s officials suggest the autism ID cards may prove useful in incidents in which a person is lost or separated from their caregiver, and is unable to effectively communicate their disability, or when the individual is nonverbal or low functioning.

The cards would clearly and quickly identify the individual as being on the autism spectrum — eliminating potential miscommunication or misinterpretation regarding behaviors that are common among those on the autism spectrum.

“It’s all about improved communication and interaction,” said Pasco Sheriff’s Cpt. Toni Roach, who helped spearhead the program through the agency’s Behavioral Health Intervention Team.

In addition to standard personal information on the cards, Roach encourages caregivers to use the additional writing space to list as much information possible about the adult or child with autism.

That information could include listing various conversation starters, preferred nicknames, and various likes and dislikes. It also could identify what triggers the individual, what calms him or her, and other useful information.

As an example, Roach explained she knows of one particular child with autism who enjoys talking about the Nintendo Switch video game console, “so the deputies would be able to see that information and go, ‘Oh, OK, I can talk about a Nintendo Switch and I can redirect the child to calm down, and build that rapport and then be able to communicate and figure out how we can best resolve that situation, and why we got called out there to begin with.”

Roach offered another example of how it could be beneficial: “Like, if that person doesn’t like to be called by their first name but likes to be called, ‘Kiddo,’ then I can refer to them by that term and that will be less confrontational with them.”

Besides receiving the autism ID cards, caregivers also have the opportunity to register their loved one’s name into the agency’s computer database, which alerts deputies they’re dealing with someone with autism.

Program attracting attention
People outside Pasco County also have expressed interest in the cards.

Roach said the cards have been sent to residents in Pinellas and Hernando counties. An officer with the Philadelphia Police Department even requested a digital version of the ID cards so his agency could implement a similar program, Roach said.

“We don’t care what county you live in, as far as I’m concerned, if we can help law enforcement interaction, we’re going to send out the card,” she said.

The autism ID cards seemingly have been embraced by members of the autism community.

Port Richey resident Candace Smith recently obtained a card for her 15-year-old son who’s on the autism spectrum. She’s also passing out some to her network of friends and neighbors who have family members with autism.

“I’m in support of anything that’s going to keep an autistic person safe, and allow them to have the same rights as everyone else,” Smith said. “I do like the cards, because it’s just an automatic way to communicate quickly to let somebody know, ‘This person has autism,’ and it explains their behavior, so to me, it’s a step in the right direction.”

The parent said she often worries what an encounter with law enforcement would entail with her son, who may not immediately be able to respond to orders from police. Rather, she explained her son’s first instinct or “self-soothing” calming behavior is usually to walk away from a stressful or nerve-inducing situation.

Those fears are further exacerbated because her son is a black teenager, Smith said. “I just worry a little bit more, because of how things are; just being realistic,” she said.

The challenging part going forward, Smith said, is finding a way to have her son understand how to correctly show the autism ID card to a law enforcement officer in a particular situation.

She acknowledged it will likely take “a series of conversations” and maybe even an in-person meeting with a deputy to get her son comfortable with the scenario.

For the time being, the parent is considering laminating the card and putting it on a lanyard for her son if he’s on his own. She plans to keep another card on her person with her son’s information, too.

University of South Florida’s (USF) Center for Autism & Related Disabilities (CARD) is likewise in support of the law enforcement agency’s autism ID card program.

The center’s assistant program director, Christine Rover, said the initiative is “a really important element” to cultivate more positive interactions between law enforcement and individuals with autism, in addition to ongoing training, outreach and collaboration with various public safety agencies.

She commended the sheriff’s office for implementing the tool and taking progressive steps with the autism community.

“Pasco County Sheriff’s Office has really been a leader in their efforts to include residents with ASD for a long time, so it’s kind of no surprise that they’re adding to sort of the elements here, and we really appreciate that,” Rover said.

Rover explained the next step is the need to develop supporting education materials and a video tutorial for caregivers to show to their loved ones with autism on how to properly and effectively present the card to law enforcement. It’s something the state-funded resource center is actively brainstorming, she said.

Meanwhile, Rover stressed autism-related training for law enforcement officers beforehand is also critical, whether it’s a case of wandering, a traffic incident or even a criminal situation.

“The training component for the deputies is really important, because where this card might be helpful on a scene, if it’s a really crisis scene, then the deputies aren’t going to have the time to really read a card and say, ‘Oh, here’s some tips for interacting,’ Rover said.

“Autism is an invisible disability, so they’re not coming onto a scene or arriving at a home and saying, ‘Oh, I can see that individual has autism,’ so they have to be able to recognize that perhaps some of those behaviors could be interpreted as part of the autism spectrum disorder.”

Therapy dog helps address mental illness

The Pasco County Sheriff’s Office has added a therapy dog to expand its efforts to better serve the needs of people facing significant mental health and substance abuse issues.

The therapy dog is the newest member of a Behavioral Health Intervention Team (BHIT) the law enforcement agency established last year.

The team’s primary task is to keep tabs on individuals who have been held involuntarily in a mental health treatment facility for up to 72 hours, through a state law known as the Baker Act.

They focus on approximately 500 people who are Baker Act repeats — through a proactive approach that includes frequent visitations, welfare checks, expedited behavioral health resources and criminal justice diversion programs.

Now, K9 Charlie, a 1-year-old pitbull-mix, has joined the team, to aid and comfort those who are struggling.

Charlie came to the local law enforcement agency by way of the Brevard County Sheriff’s Office’s Paws and Stripes program.

That program trains dogs from local animal shelters, preparing them to become PTSD dogs,  therapy dogs, and child victim advocate dogs.

Charlie was rescued from the streets in October.

The new animal assisted therapy dog program strives to better “build a connection” between law enforcement and the mental health community, said Cpt. Toni Roach, who oversees the 15-member BHIT, which includes a captain, two sergeants and 12 detectives.

Charlie helps “to comfort and ease some anxiety and build that stress relief,” when the sheriff’s office comes knocking on the door for visitations and welfare checks, Roach explained.

“Law enforcement and people with mental illness, there’s that stigma that we’re there to take them into custody or arrest them for a crime,” Roach said.

But, Charlie’s calming presence for someone facing a mental health crisis could break the ice, Roach said. And, that can help to open lines of communications, so responders can get to the root of problems and identify ways to resolve them.

The Pasco Sheriff’s Office Behavioral Health Intervention Team (BHIT) recently added animal assisted therapy dog Charlie to the team. Charlie is partnered with BHIT Det. Pedro Leos, and aims to provide a calming presence for those that may be experiencing a mental health crisis.

An individual struggling with addiction may be referred to outpatient substance abuse treatment, for instance. Or, someone undergoing financial struggles may be referred to Pasco County Human Services and the county’s homeless coalition.

Roach said the idea is to help bridge the gap for services and shorten the time it takes to receive them.

A therapy dog can help in that process, she said.

Charlie’s handler, Det. Pedro Leos, agrees.

Leos said the therapy dog was needed to “better help me make contact with those people in need, break down those barriers between law enforcement and the community, and open up conversation.”

Leos has been with the sheriff’s office in 2014. He said he joined the BHIT “because there’s a stigma with mental illness and I want to break that cycle.

“I want to be there to help people in need, and give them the resources they need to have a better quality of life in order to continue on with their daily activities,” the detective said.

When approached a few months ago to become a therapy dog handler, Leos, who loves pets, said he “was all for it.”

Leos noted there was an immediate connection between him and Charlie during the eight-week Paws and Stripes training program.

“When I met him, I absolutely loved him,” Leos said. “It was like we clicked. He listened to me. We started doing obedience training, and he was awesome. It was just one of those things where we both bonded very quickly.”

The detective said every time he puts on his uniform — a black polo and green spruce pants — Charlie knows it’s time to go to work “and help people in the community.”

Though still a young canine, Charlie has already settled into his role, Leos said.

The detective described Charlie’s demeanor this way: “He’s awesome with kids, awesome with people, awesome with the community. He loves hugs and he loves kisses. He loves to make people happy, and he knows that’s his main goal, to make people happy. He understands it, and he’s out there doing what he needs to do, to help fulfill this desire to help people in need.”

The sheriff’s office plans to add additional therapy dogs at some point, so that at least one can be available seven days a week, officials say.

Symposium seeks addiction recovery solutions

A symposium is scheduled for later this month that will seek to create a blueprint for improving long-term substance abuse recovery efforts in Pasco County.

The Recover Out Loud Pasco Symposium is set for Sept. 28 from 9:30 a.m. to 2 p.m., at Calvary Chapel Worship Center, 6825 Trouble Creek Road in New Port Richey.

Admission is free.

The symposium is being organized by Recovery Epicenter Foundation, a newly licensed and accredited recovery community organization operating out of Trinity.

The symposium will seek to engage individuals, stakeholders and community leaders in an effort “to grow community-oriented initiatives, cultivate community allies, and develop recovery peer-based programs,” according to the event’s flyer.

Its keynote speaker is Dona Dmitrovic, executive director of Las Vegas-based Foundation for Recovery and former National Director of Consumer Affairs for Substance Use Disorders at Optum, United Health Group.

Other noted speakers include:

  • Tina Levene, motivational speaker and published author on abuse and addictions
  • Pasco Sheriff’s Office Lt. Toni Roach, who heads the agency’s Crisis Intervention Training (CIT) and Mental Health and Threat Assessment (MHAT) teams
  • Sara Mollo, Sixth Judicial Circuit chief assistant public defender
  • Former Sixth Judicial Circuit Court Judge Lynn Tepper
  • Sarah Cobelli, Baycare Behavioral Health program manager
  • Ginny LaRue, Florida Alcohol & Drug Abuse Foundation recovery project director

Additionally, the symposium will feature interactive café conversations about which recovery-related services are working in the community and what needs more support, along with a series of panel discussions from people in recovery and existing recovery community organizations throughout Florida.

Thoughts and input gathered from the symposium will be incorporated into the Recovery Epicenter Foundation’s visioning process in November to better define action steps for recovery-based programming, said Recovery Epicenter Foundation RCO director Rachel Starostin.

“The whole thing is we want to get the community together,” said Starostin, who expects anywhere from 200 to 300 attendees at the symposium. “The community benefits when we all collaborate together.”

Starostin said the Recovery Epicenter Foundation’s “ultimate goal” is to build a peer-run, recovery-oriented community center to facilitate programs in the way of employment assistance, transportation, health and wellness, recreational opportunities, independent support groups and so on.

“We’re like building a shell out of something that needs to exist. We’re building something out of nothing,” she said.

“It’s kind of an abstract concept, when you don’t have a building and trying to plan out the future,” she said.

Starostin emphasized the need for such a recovery facility to help people “rebuild their lives” once they get out of addiction treatment. That assistance may initially include assisting them with “locatable, tangible resources,” such as basic medical care, living shelters, food stamps and more.

Starostin put it like this: “As a general rule, until you meet somebody’s basic needs, they’re not really receptive to listen. Like, how well do you focus when you’re hungry and you haven’t eaten in two days?”

Meanwhile, Starostin said the upstart recovery community organization is looking to mobilize more peer specialists to help with implementing various recovery programs. Peer specialists, or peer coaches, are people who have been successful in the recovery process who help others battling addiction through shared experience, understanding, respect and mutual empowerment.

Said Starostin, “There’s something magical when one peer who’s suffered from substance use works with another. You identify with them, you trust them, and it’s not like a 12-step program, you help somebody kind of come up with their own plan.”

Recovery Epicenter Foundation’s recovery community organization operates as an independent, nonprofit entity — led and governed by representatives of local communities of recovery.

Its mission is to support recovery through advocacy, education and peer-to-peer support services, to improve outcomes for persons seeking drugs and alcohol addictions.

Guidance and technical assistance on the Recovery Epicenter Foundation recovery community organization’s development process is coming from the Florida Recovery Project/Faces & Voices of Recovery, with collaboration from Florida Alcohol & Drug Abuse Foundation, Florida Department of Children and Families, and Peer Support Coalition of Florida.

An initial community listening session for the recovery community organization was held in February at the Land O’ Lakes Community Center, followed by a symposium planning session.

The sessions have already brought together dozens of stakeholders, including persons in recovery and professionals in health care, criminal justice, faith-based organizations and others.

“When we started this process, there wasn’t a whole lot of momentum behind it, but we’re getting it; people are getting involved and they’re starting to understand,” Starostin said.

Pasco Sheriff launches unit to help mentally ill

The Pasco County Sheriff’s Office is launching a new unit aimed to better serve the needs of people facing significant mental health issues.

The new unit — called the Mental Health and Threat Assessment Team (MHTAT) — will feature six deputies, two caseworkers, a clinical social worker, a sergeant and a lieutenant, who will collaborate with local behavioral health providers to provide tailored, long-term programs for citizens in need.

The team’s primary task is to keep tabs on the county’s Baker Act repeats — through a proactive approach that includes frequent visitations, welfare checks, expedited behavioral health resources and criminal justice diversion programs.

An individual struggling with addiction may be referred to outpatient substance abuse treatment, for instance. Or, someone undergoing financial struggles may be referred to Pasco County Human Services and the county’s homeless coalition.

The unit will have partnerships with BayCare Behavioral Health, Chrysalis Health, Novus Medical Detox Center, HCA Florida Hospitals, AdventHealth and others, “working towards a common goal in our community,” Pasco Sheriff Chris Nocco said.

The program ultimately will put the agency “ahead of the curve” in crime prevention, the sheriff said.

“We see mental health and substance abuse are the two drivers of criminal justice issues in our county. That’s why we’re creating this unit,” Nocco said.

It’s also about crisis mitigation, said Lt. Toni Roach, who will head up the MHTAT unit.

“Everybody has a baseline, and when they start to dip below that baseline we can provide some intervention strategies, connect them with a case manager or whatever other behavioral health resources are in the community that could help them stabilize,” Roach said.

The unit is expected to be up and running by October. It will cost roughly $1.5 million annually.

About 11 percent of the sheriff’s calls for service in 2018 were mental-health related.

That included roughly 3,400 Baker Act reports and more than 2,100 calls involving suicides or suicide attempts.

Of those reports, 503 individuals had multiple interactions with the agency, including some who’ve been Baker Acted as many as four or five times, Nocco said.

The MHTAT will be concentrating on the population who have had multiple interactions with the county, Nocco said. The unit will help divert those people from having to call 911 and thereby free patrol deputies to respond more quickly to urgent or violent calls.

As an example, the sheriff pointed out that, last year alone, one individual with a history of mental illness called county dispatch 124 times.

But, through a personal visit from the sheriff’s office back in May, those calls have stopped, the sheriff said.  “A lot of times they just need to talk to somebody. They just need somebody to help them out.”

“It’s all about connection,” added Roach. “Interacting with anybody is just that communication piece, being able to sit down with somebody and have a conversation with people, to listen to what’s going on, what are their concerns, what are their barriers.”

And, it’s those types of soft skills that will be required for those selected to the 11-person unit.

“It takes a special person to want to be in this unit,” Nocco said. “You want somebody that has that compassion and care. Somebody who says, ‘I’m not just going to be here for an hour, I might be here two to three hours working with somebody.’”

In recent years, the sheriff’s office has placed an emphasis on training law enforcement personnel to respond better to people who are mentally ill.

The office has an eight-hour foundational course in mental health first aid and a 40-hour Crisis Intervention Training (CIT) program held quarterly in Shady Hills. About 50 percent of the agency’s patrol deputies are CIT certified.

Pasco Sheriff’s Office hone crisis intervention skills

Pasco County Sheriff’s Office deputies recently spent a week learning how to use the power of words to de-escalate situations that have the potential to turn dangerous.

In one role-play scenario, Pasco Sheriff’s Office deputies Breanna Chandler and Lindsay Steward intermediated between a mother and her teenage daughter with oppositional defiant disorder.

Other mock scenarios had deputies and corrections officers encounter inmates or juveniles with suicidal thoughts, or, deal with someone with severe substance abuse. There was even a situation that exposed deputies to someone with low-functioning autism.

There weren’t any takedowns, hands-on combat nor use of nonlethal force.

Rather, this type of police training simply required conversations to resolve problems.

For each participating law enforcement officer, the objective was to have a better understanding of mental health in crisis and to learn how to better respond to de-escalate a situation during a call for service.

It was all part of the agency’s recent Crisis Intervention Team (CIT) training program at Pasco Safety Town in Shady Hills.

The weeklong, 40-hour CIT program is a collaborative effort between the sheriff’s office and many local behavioral and mental health community partners. It is held four times per year within the agency.

Throughout the week, deputies received instruction on various subjects, such as signs and symptoms of mental illness, co-occurring disorders, addiction, dementia, trauma informed care, and juvenile and veteran response.

De-escalation techniques centered on maintaining positive body language and tone of voice, along with active listening skills to appropriately reflect upon a subject’s feelings.

Deputies then put those techniques to the test with role-play scenes, which were written out by a licensed mental health counselor in collaboration with a law enforcement officer. Volunteer actors played the various roles of persons in crisis.

The mock exercises required deputies to determine whether or not to detain a subject for an involuntary mental health evaluation, otherwise known as the Baker Act; provide a referral to various behavioral health services and resources; or, simply offer someone a shoulder to lean on and to listen to a subject’s personal problems.

One of the instructors and role play actors was Tom Kelley, a former law enforcement and military officer, who operates a consulting firm specializing in crisis intervention and crime prevention.

In traditional law enforcement academies, trainees are taught to enter situations with authority in voice and action.

In the crisis intervention program, however, Kelley instructed deputies to find a softer, more inviting tone when dealing with people undergoing a mental crisis.

Kelley put it this way to a classroom full of deputies: “Let them know that you’re there and you care. At that moment, they need to feel like you’re the most caring person in the world, because they are at their worst.”

Pasco Sheriff’s Lt. Toni Roach has been the agency’s CIT coordinator since 2016.

She said the objective of crisis intervention is twofold.

First, it’s to reduce potential injury to citizens and responding deputies.

Second, it’s utilized to help divert people with “potentially criminal behavior or disruptive behavior” away from the jail system and into a mental health system where they can get proper diagnosis and treatment.

Roach explained: “The idea is to use your words and de-escalate rather than quickly resorting to force when the people you’re interacting with become nervous or scared, and are apprehensive to do what the deputy asks them to do.”

The training is vital for law enforcement personnel, the lieutenant said.

Roach put it this way: “We want to make sure that we can serve our public and provide them with good resources, and then hopefully we interact with them in a positive manner, so that, especially people with mental illness who have a fear of law enforcement, we can reduce that level of fear and have a good relationship with our community.”

Twenty-nine sheriff’s office personnel graduated from the recent spring class.

Upon graduation, each deputy received a pin for their uniform indicating they’ve had advanced training in crisis intervention.

Roach said about 50 percent of patrol deputies are CIT certified, also noting the public is able to request a CIT trained officer in a call for service. The agency is striving to have all of its deputies CIT certified in the future, she said.

In addition to the sheriff’s office, organizations that assisted in facilitating the CIT program were the National Alliance on Mental Illness, HCA Florida Hospitals, Rogers Behavioral Health, North Tampa Behavioral Health, BayCare Health System, Gulf Coast Jewish Family and Community Services, and Veteran’s Affairs clinics.
